Classified Staff Terminating Employment

If you are terminating employment with Virginia Commonwealth University (VCU), please review the following important information concerning the status of your current benefits and complete the forms as indicated.  Click on the links to print the forms (unless otherwise indicated).  A termination packet, which includes these forms, also is available from VCU Human Resources - Lindsey House Quick Stop at 600 West Franklin Street. 

You may convert your group life insurance to an individual whole life policy with Minnesota Life. If you wish to convert your policy without having to complete a medical questionnaire, return this completed form within 31 days of your employment termination date.

Please Note:  This form is not available on line -- you will need to contact Minnesota Life at 1-800-441-2258 or VCU Human Resources. 

If you are a member of the Virginia Retirement System (VRS), you may request a refund of your retirement contributions, rollover your contributions to a qualified retirement plan, or keep them with the VRS.  The VRS Factsheet provides information on your options and the tax consequences of taking a distribution.

Complete and return this form to request a refund or rollover of your VRS contributions.  You must include with this form a copy of your Social Security card or other official document that shows your Social Security number.

Complete and return this form if you would like your retirement contributions to remain with VRS. 

If you currently participate in the U.S. Savings Bonds Program, contact the National Bond and Trust Company (NBT), VCU's third-party vendor, at 1-800-426-9314 or visit the NBT website at http://www.nbtco.com/.

Complete and return this form if you would like VCU to provide your University employment information upon request.

Complete and return this form to let us know if you have a change of address.
